When the students reach the age of 14 (Year 9), planning for transition will be discussed with the students and parents at their Annual Review meeting.
Throughout the pupil’s final years of study, a programme will be designed to encourage them to think about and experience the various options that may be available to them.
At the pupil’s final Transition Review, options are discussed for the next placement.
This may include options offered by Kisharon Adult Services such as Hackney Community College and supported living, as well as other community organisations and local Adult Education providers.
Once a decision has been reached as to the next placement, we will work together with the new provider to ensure a thorough and detailed transition occurs.
